The Consent Economy: Patients as Owners of the Data Supply Chain

The thesis

Health data powers every model, insight and cure — and the people who produce it, the patients, are cut out of its economy entirely. Their data is harvested, brokered and monetised while they get a privacy notice they didn't read. This is not just unjust; it is why trust — and data quality — are collapsing.

Build the consent economy

Flip patients from product to owner of their data supply chain:

  • Ownership. The patient holds the master record and the keys.
  • Granular consent. They license specific data, for specific purposes, for specific durations — revocable at any time.
  • A dividend. When de-identified data creates value, the patient shares in it. Contribution earns return.
  • Transparency. Every use is logged and visible to the person it describes.

Why this makes the data better

  • Trust compounds. People who are owners, not victims, contribute more and richer data.
  • Bias shrinks. Fair, paid participation pulls in the populations that opt out of extractive systems.
  • Consent is auditable, so researchers and regulators can prove provenance.

The mechanism already exists

Consent-first cohorts, de-identification, revocable licences and a data dividend are engineering problems we know how to solve. What's missing is the decision to treat patients as principals in their own economy.

Provocation

Every other industry learned that the people who create the value should share in it. Medicine's data economy is the last great extraction that still pretends otherwise. Fix the ownership and trust, quality and participation all rise together.
